Claims should be filed with the Clerk of the U.S. Bankruptcy Court in Wichita, KS. Completed claim forms, including copies of supporting documentation, can be mailed to the Clerk, filed electronically on the Court's CM/ECF system by authorized filers, or filed in person at:

To receive a copy of the claim for the creditor’s records after it is filed, the original signed and dated claim, plus a copy, must be filed with the Clerk. If mailed, a self-addressed, stamped envelope must be provided to the Clerk.
