A complete set of standard Bankruptcy Forms is available on the U.S. Courts Bankruptcy Forms page at http://www.uscourts.gov/forms/bankruptcy-forms. Generally, the Bankruptcy Court for the District of Kansas uses the Official Bankruptcy Forms. There are, however, a few instances where the Court uses locally customized forms. For your convenience, the following list contains only the forms that have been locally customized. If a given form does not appear in this list, you should use the Official Bankruptcy Form that appears on the uscourts.gov website.
On December 1, 2015, all of the Bankruptcy Forms were updated. Some forms were abrogated, some were new, and others were modified substantively or cosmetically. All forms were renumbered. To help you view at a glance how the new forms correspond to prior versions, we provide a Modernized Bankruptcy Forms Numbering Conversion chart.
Cases filed on or after April 1, 2016, should use certain forms that became effective on that date. On April 1, 2016, nine bankruptcy forms (and several instructions) were updated to reflect the dollar amount adjustments that are calculated at three-year intervals based on the Consumer Price Index.
